Pediatricians Split on Heart Tests Before Kids' ADHD Meds (HealthDay)

MONDAY, Jan. 16 (HealthDay News) -- Some pediatricians continue to do electrocardiograms (EKGs) on children before starting them on medication for attention-deficit hyperactivity disorder, new research suggests, even though many experts say the latest evidence shows it isn't really necessary.

Several years ago, reports of sudden death, heart attack and stroke among children and adults taking stimulants to treat ADHD caused alarm among parents and health care providers about the safety of the medications.

The reports prompted Canadian health authorities to briefly pull Adderall from the market in 2005, while the U.S. Food and Drug Administration now requires that ADHD drugs carry a "black box" label warning about potential heart risks.

Further research suggested that the risk may only be to children who had underlying heart defects, such as some congenital abnormalities and arrhythmias. On Adderall, for example, the warnings now read: "Sudden death has been reported in association with CNS [central nervous system] stimulant treatment at usual doses in children and adolescents with structural cardiac abnormalities or other serious heart problems."

A few years ago, the American Heart Association stated that it would be "reasonable" to give kids EKGs, which look for abnormalities in the heart's electrical activity, before starting them on stimulant medication.

But in 2008, the American Academy of Pediatrics (AAP) issued a policy statement saying that routine EKGs prior to starting kids on ADHD medications wasn't necessary.

"A lot of pediatricians started doing EKGs, and then when the AAP said it didn't agree with that, pediatricians scaled back," explained Dr. Andrew Adesman, chief of developmental and behavioral pediatrics at Steven & Alexandra Cohen Children's Medical Center of New York.

And two major recent studies have found no hearts risk associated with ADHD medications. In a study published last November in the New England Journal of Medicine, researchers concluded that medications such as Ritalin, Adderall and Concerta don't raise the risk of sudden death, heart attack or stroke in children and young adults.

In the study, researchers from Vanderbilt University and colleagues analyzed data on 1.2 million children and young adults aged 2 to 24 enrolled in four large health plans around the United States.

And a study published last December in the Journal of the American Medical Association analyzed data on more than 150,000 young and middle-aged adults taking one of several ADHD drugs and found no added risk of heart attack, sudden cardiac death or stroke, even among people with a family history of heart disease.

"The recent data suggests there is no increase in sudden cardiac death or any need for cardiac monitoring, provided there is no history of heart disease in the patient and no family history of heart disease," said Dr. Victor Fornari, a professor of psychiatry at Hofstra North Shore-LIJ School of Medicine, in New York.

The current research, published in the February issue of Pediatrics, surveyed 525 U.S. pediatricians about their ADHD prescribing practices.

The vast majority of physicians said they did a routine medical history and physical before putting kids on stimulants to treat ADHD.

Less than half (48 percent) did a more in-depth cardiac history and physical, which is recommended by the AAP.

About 15 percent also ordered an EKG to look for abnormal heart rhythms. The most common reason for continuing to do EKGs was that it was the "prevailing practice" where they worked.

"Pediatricians have a very variable attitude toward the safety and efficacy of these medications," Fornari said. "Even though there is no evidence base to suggest continued cardiac screening, there persists this lingering attitude among physicians that 'I'm not comfortable unless I do it'."

And even many children with underlying heart issues are safely taking ADHD medications, Adesman said.

"Most children with underlying heart problems are likely still eligible to be treated with stimulants once the family, the pediatrician and a consulting pediatric cardiologist agree that it's justifiable and safe," Adesman said.

He added: "There are only a few very specific, relatively uncommon cardiac problems where one would be hesitant to use a stimulant."

The survey also found nearly half (46 percent) of pediatricians said they also discussed stimulant-related heart risks with parents. In most situations, making sure families are fully informed of any risks is good practice, Fornari said. But given how little evidence there is that ADHD medications pose heart risks to children, in this situation those conversations may be causing unnecessary worry and deterring some families from trying stimulants.

Almost 3 million children in the United States take prescription medications for ADHD each year. Children with the neurobehavioral disorder have excessive levels of activity, inattention and impulsiveness.

Pakistan PM won't retract criticism of military (Reuters)

ISLAMABAD (Reuters) ? Pakistan's prime minister on Sunday rejected a demand by the country's powerful army chief that he clarify or retract his criticism of the army and the spy agency last week, likely raising tensions further in a festering row with the military.

"The prime minister ... is answerable to parliament," Yusuf Raza Gilani told reporters in the central city of Vehari. "I will not answer to a person. I am answerable to parliament."

Recent tension has raised fears for the stability of the nuclear-armed country and exposed a struggle between the government and the military, which has ousted three civilian governments in coups since independence in 1947 and has ruled the nation for more than half of its history.

Gilani last week criticized Army Chief General Ashfaq Kayani and the director general of the Inter-Services Intelligence agency Lieutenant-General Ahmed Shuja Pasha for filing court papers in a case involving a mysterious memo that has pitted the military against the civilian government.

In an interview with Chinese media, Gilani said the filings were "unconstitutional," infuriating the military's high command, who issued a stern press release on Wednesday.

"There can be no allegation more serious than what the honorable prime minister has leveled," it said.

"This has very serious ramifications with potentially grievous consequences for the country."

Gilani further infuriated the army on Wednesday by sacking the defense secretary, retired Lieutenant General Naeem Khalid Lodhi, for "gross misconduct and illegal action which created misunderstanding" between institutions.

Lodhi was the most senior civil servant responsible for military affairs, a post usually seen as the military's main advocate in the civilian bureaucracy.

The unusually public sniping comes amid a roiling political scandal involving the mysterious memo.

The memo, allegedly drafted on the direction of former ambassador to Washington Husain Haqqani, asked for U.S. help in reining in the army, which the memo said was planning a coup.

When an American businessman revealed his role in writing and delivering the memo, the army was enraged. Haqqani was forced to resign, and "memogate" has locked President Asif Ali Zardari and the military in trench warfare ever since.

DIVISIVE STATEMENTS

Gilani's comments were in response to a journalist's question about media reports Saturday night that Kayani was infuriated by Gilani's criticisms.

The army chief complained to Zardari and demanded that Gilani's comments be clarified or withdrawn, a military source told Reuters on Saturday.

Gilani, however, showed no signs of backing down.

"What I said was not an accusation," he told reporters. "We want there to be respect for the constitution, rule of law, and all institutions to work within their limits. I said just one thing, that rules and procedures were not followed. And that was the defense secretary's fault, for which we removed him from his post."

The military, despite being officially under civilian control, sets foreign and security policies and drew rare public criticism after U.S. special forces killed al Qaeda leader Osama bin Laden on Pakistani soil in a raid in May 2011, an act seen by many Pakistanis as a violation of sovereignty.

Pakistanis rallied behind the military after a November 26 cross-border NATO air attack killed 24 Pakistani soldiers on the frontier with Afghanistan, driving ties with Washington to their lowest point in years.

The latest crisis also troubles Washington, which wants smooth ties between civilian and military leaders so that Pakistan can help efforts to stabilize neighboring Afghanistan, a top priority for President Barack Obama.

Six more killed in Syria despite Arab monitors (Reuters)

BEIRUT (Reuters) ? Five civilians were killed in Syria and a brigadier was assassinated on Monday in violence that has not abated despite an Arab peace plan monitored by Arab League observers.

Arab League foreign ministers will meet on Sunday to discuss the future of the mission sent last month to check if Syria is abiding by the Arab plan it accepted on November 2.

The plan required Syria to halt the bloodshed, withdraw the military from cities, free detainees and hold a dialogue.

Hundreds of people have been reported killed in Syria even since the monitors deployed on December 26 as forces loyal to President Bashar al-Assad try to crush peaceful protests that began 10 months ago, as well as armed resistance to his rule.

Random gunfire from pro-Assad militiamen killed five people, including a woman, and wounded nine in the restive city of Homs, the British-based Syrian Observatory for Human Rights said.

The state news agency SANA said an "armed terrorist group" had shot dead Brigadier-General Mohammed Abdul-Hamid al-Awad and wounded his driver in the countryside near Damascus.

The latest violence erupted a day after U.N. Secretary-General Ban Ki-moon told Assad to "stop killing your people."

Assad's harsh response to the uprising has killed more than 5,000 people, by a U.N. count. The Syrian authorities say 2,000 members of the security forces have also been killed. The deaths of 32 civilians and soldiers were reported on Sunday.

"STOP KILLING YOUR PEOPLE"

"Today, I say again to President Assad of Syria: stop the violence, stop killing your people. The path of repression is a dead end," Ban told a conference in Lebanon on Sunday.

The head of the Arab monitoring mission is due to report to an Arab League committee on Thursday, ahead of a wider meeting of Arab foreign ministers to consider their next step on Syria.

Qatar, which heads the League committee on Syria, has suggested Arab troops step in to stop the killing, an idea which is anathema to Damascus and which several Arab countries, including Iraq, Lebanon and Algeria, are likely to oppose.

An Arab representative to the Cairo-based League said it had received no formal proposal for such military intervention.

The League also has the option of referring Syria to the U.N. Security Council, which has so far failed to take any action due to opposition from Russia and China to any resolution that could lead to U.N. sanctions or Western military action.

There is little Western appetite for any Libya-style intervention. The United States, the European Union, Turkey and the Arab League have announced economic sanctions against Syria.

Assad proclaimed an amnesty on Sunday for "crimes" committed during the uprising and some detainees were later freed in the presence of Arab monitors in Damascus.

Kinan al-Shami, of the Syrian Revolution Coordination Union, said hundreds of detainees appeared to have been released, but they represented only a fraction of at least 40,000 people he said had been jailed without charge since March, many of whom have been held in secret police buildings or makeshift prisons.

Among those freed, Shami said, was Syrian actor Jalal al-Tawil who was shot and captured while trying to flee to Jordan two weeks ago. He had earlier been beaten in a Damascus protest.

Assad has issued several amnesties in recent months, but opposition groups say thousands of people remain behind bars and many have been tortured or abused, with some killed in custody.

The movement to end more than four decades of Assad family rule began with largely peaceful demonstrations, but after months of violence by the security forces, army deserters and insurgents started to fight back, prompting fears of civil war.

Assad, who retains the support of core military units, is backed by his own Alawite minority as well as some minority Christians and some majority Sunni Muslims who fear chaos, civil war and the rise of Islamist militancy if he is toppled.

The president, 46, who appeared in public twice in as many days last week, is eager to show that his people love him.

SANA, the state news agency, reported on Sunday that a 10 km (six mile) long letter, which it billed as the world's longest, was being written and signed by Syrians across the country as a "message of loyalty to the homeland and its leader."

Blocking cell-to-cell communication may prevent liver damage and improve drug safety

Massachusetts General Hospital (MGH) investigators have developed a novel strategy to protect the liver from drug-induced injury and improve associated drug safety. In their report receiving advance online publication in the journal Nature Biotechnology, the team reports that inhibition of a type of cell-to-cell communication can protect against the damage caused by liver-toxic drugs such as acetaminophen.

"Our findings suggest that this therapy could be a clinically viable strategy for treating patients with drug-induced liver injury," says Suraj Patel, PhD, of the MGH Department of Surgery, the paper's lead author. "This work also has the potential to change the way drugs are developed and formulated, which could improve drug safety by providing medications with reduced risk of liver toxicity."

Developing, approving and prescribing a drug requires that the therapeutic benefits be weighed against any potential toxicities. Liver toxicity limits the development of many therapeutic compounds and presents major challenges to both clinical medicine and to the pharmaceutical industry. Drug-induced liver injury is the most common cause of acute liver failure in the U.S. and is also the most frequent reason for abandoning drugs early in development or withdrawing them from the market. Since no pharmaceutical strategies currently exist for preventing drug-induced liver injury, treatment options are limited to discontinuing the offending drug, supportive care and transplantation for end-stage liver failure.

Gap junctions are hollow channels that connect neighboring cells and allow direct intercellular communication between coupled cells. In the heart, gap junctions are known to propagate the electrical activity required for contraction, but their role in the liver is poorly defined. Recent work by the MGH team and others has shown that assemblies of intercellular gap junctions spread immune signals from injured liver cells to surrounding undamaged cells, amplifying overall inflammation and injury. The current study was designed to discover the potential of targeting liver-specific gap junctions to limit drug-induced liver injury.

The researchers first used a strain of genetically mutated mice that lack a particular liver-specific gap junction. The mice were administered various liver-toxic drugs, such as the commonly used medicine acetaminophen. Overdoses of acetaminophen, which is best known under the brand name Tylenol, are the most frequent cause of drug-induced liver injury. Compared to normal mice, those lacking liver gap junctions were protected against liver damage, inflammation and death caused by administration of liver-toxic drugs.

The team then identified a small-molecule inhibitor of liver gap junctions that, when given with or even after the toxic drugs, protected the livers of normal mice against any injury and prevented their death. Additionally, cell culture experiments indicated that blocking gap junctions limited the spread through liver cells of damaging free radicals and oxidative stress, suggesting a possible mechanism for the observed protection.

"This finding is very exciting and potentially very powerful from a number of basic science and clinical application standpoints, which we are continuing to explore," says Martin Yarmush, MD, PhD, director of the MGH Center for Engineering in Medicine and senior author of the study. "However, before we can think about applying this approach to patients, we need to know more about any off-target effects of these gap junction inhibitors and better understand the long-term ramifications of temporarily blocking liver-specific gap junction channels."

A patent related to the work has been filed by Partners Healthcare, and an early stage biotechnology company, Heprotech Inc., was recently established to develop this new technology further. "The findings from this work suggest a novel drug development strategy in which therapeutically effective but potentially liver-toxic compounds could be co-formulated with selective gap junction inhibitors to improve their safety," explains Patel, a co-founder of Heprotech along with Yarmush. "We look forward to helping commercialize this new technology, with the ultimate goal of developing liver-safe pharmaceuticals and better treatments for drug-induced liver injury."

MSI focuses notebook business on gaming models

Micro-Star International (MSI) has focused its notebook business on more profitable gaming models because consumer models are already in keen competition, according to senior vice president of MSI Henry Lu.

Lu pointed out that the company plans to conduct a new wave of personnel adjustment in the future, but he did provide any time schedule for the plans.

Sources from the notebook industry pointed out that MSI's notebook shipments in 2011 decreased by at least 60% from 2010, while its all-in-one PC shipments in 2011 were also 70% below its original target, causing MSI to start reconsidering its personnel and product line up strategies.

Compared to the notebook business, MSI's motherboard business continues to perform steadily, while its graphics card business also seeing improvements in both shipment volume and ASPs, with ASPs even having a chance to break US$100, an increase of about 10%.

However, affected by hard drive shortages, MSI's motherboard shipments are expected to drop in the first quarter with affects to start dissipating in the second quarter.

Apple discloses suppliers for the first time (AP)

NEW YORK ? Apple has disclosed a list of its suppliers for the iPhone, iPad and other popular gadgets for the first time amid growing criticism over labor and environmental practices, especially in China.

The list for years has been strictly and fervently protected. Even a glimpse of an Apple iPhone or its components before a public release became a source of intrigue and controversy. Investors have played a guessing game about which contractors would become Apple's next supplier, or who was on the outs.

Apple disclosed its list of its suppliers responsible for 97 percent of its procurement expenditures worldwide. Some examples are Intel Corp., Broadcom Corp., Amphenol Corp. and Sanyo Electric Co.

The transparency may be a sign of changes coming to the Cupertino, Calif. company in the post-Steve Jobs era under CEO Tim Cook.

The 2012 "Supplier Responsibility Progress Report" released Friday, documents 229 audits throughout its supply chain last year by Apple Inc. That's up 80 percent from 127 audits in 2010.

The audits found labor, health, health and environmental violations, including instances of underage labor and discrimination based on pregnancy. Apple also outlined its response to each of the violations that were uncovered, which included ending its relationship with repeat offenders and requiring companies to come up with measures to prevent them from occurring again.

The report was issued a day after distraught workers who make Microsoft's Xbox video game consoles at Foxconn Technology Group climbed to the top of a six-story dormitory and threatened to jump to their deaths. No one did, but the incident highlights growing labor unrest in China. Foxconn is a unit of Taiwan's Hon Hai Precision Industry Co. that makes iPads and iPhones for Apple.

There was a rash of suicides at the massive Foxconn plant in 2010 in the city of Shenzhen. About 300,000 people work at the plant and industrial park. Plant managers installed nets to prevent more people from committing suicide by jumping from the roof.

In its report, Apple found that 78 of the company's suppliers complied with antidiscrimination standards it has set for them. But only 61 percent of them had systems in place to prevent discrimination from happening.

Nearly all ? 97 percent ? prevented underage labor. But only 72 percent had policies in place keep it from happening.

Just 38 percent of the suppliers observed Apple's working-hours policies, and 69 percent followed its code for wages and benefits.

In all, the audits found 74 percent of the suppliers to be in overall compliance with Apple's policies. Examples of violations included testing job candidates for Hepatitis B, conducting pregnancy tests and exceeding weekly limits of 60 working hours. Apple said 109 facilities it audited did not pay proper overtime wages.

In another shift, Apple also joined the Fair Labor Association, a group of companies and universities focused on improving labor practices. It conducts unannounced, random audits on its members' factories.

Showtime making documentary about Dick Cheney (AP)

PASADENA, Calif. ? The Showtime television network says it is preparing a new documentary on former Vice President Dick Cheney.

Showtime said Thursday that filmmaker R.J. Cutler, who made "The War Room" about Bill Clinton's presidential campaign in 1992, will make the film. No date was set for airing.

Cutler calls Cheney, who was George W. Bush's vice president, "perhaps the single most influential non-president in the nation's political history." He promises a balanced and multi-dimensional look at Cheney.

Showtime says it has begun an effort to produce high-end profiles. Its other project is about rap mogul Suge Knight.
